Health and Hospital Corporation of Marion County (“HHC,” “Corporation,” “we,” or “us,”) is committed to protecting your privacy and ensuring that your information is managed in a responsible manner. This
Privacy Policy explains what personal information we collect from you or about you through our
websites and mobile applications, and how we manage that information. Our Privacy Policy applies to MyMCPHDChart-app.marionhealth.org and MyMCPHDChart, and all related domains (collectively, “HHC applications”). By accessing, browsing, and/or using our applications, you are deemed to have accepted the terms of this Privacy Policy.
What Information Do We Collect?
When you visit our applications, we may collect your personal information, such as your name, email and mailing address, phone number, and any correspondence you submit when you contact us via message or e-mail. In some circumstances, we may request your username and password when you
create an account for our patient portal, and capture details of your use of our portal. In most instances, Personal Health Information (“PHI”) may be collected, which includes information about your current health conditions, treatments, medicines, symptoms, and payments for health care.
Information We Collect Through Automatic Data Collection Technologies
As you navigate through and interact with our Platform, we may use automatic data collection technologies to collect certain information about your equipment, browsing actions, and patterns, specifically:
· Platform Activity. Details of your visits to our Platform such as traffic data, Location Data, logs, referring/exit pages, date and time of your visit to our Platform, error information, session replay and recording data, browsing activity, clickstream data, and other communication data and the resources that you access and use on the Platform;
· Location and Appointment Data. Our Platform uses Location Data to prompt users to check in for scheduled services when they are near their service location. Prior to our use of location data, users need to consent and can opt out at any time. And
· System and Device Data. Information about your computer, mobile device, device measures, and Internet connection, specifically your IP address, operating system, browser type, and App version information.
The information we collect automatically may include Personal Data or we may maintain it or associate it with Personal Data we collect in other ways or receive from third parties. It helps us to improve our Platform and to deliver a better and more personalized service by enabling us to:
· estimate our audience size and usage patterns;
· store information about your preferences, allowing us to customize our Platform according to your individual interests;
· recognize you and your location to provide you with appointment data when you return to our Platform.
How Do We Use Your Information?
If you choose to provide HHC with your personal information– for example, by creating an account
through our website, submitting a “Contact Us” form, or sending an email – we may use that information to respond to your message, help you get the services you requested, improve the quality of our service, or comply with legal requirements.

· When we have your permission to share with your family or trusted friends to engage in your care and wellbeing.

Where Does Your Information Go?
We may share the information you provide with trusted partners, including our affiliates and third-party service providers, who work on our behalf to provide health care services. For any business partner, HHC will maintain contracts restricting their access, use, and disclosure of personal information in compliance with this Privacy Policy and any legal obligations.

Some of our websites may use a technology called “cookies.” A cookie is a very small file placed on your hard drive by a Web page server. It is your identification card and can help us analyze traffic to our
